DJO® Corporate Compliance and Finance have not necessarily reviewed and approved any of the courses on the 2020 calendar or the engagement of any HCP to assist with these courses. Per
DJO Policy, no DJO Surgical® employee may make any commitment, verbal or otherwise, to any third party (including HCPs) – whether that third party is providing products, services, or facilities

In accordance with the Physician Payments Sunshine Act, DJO must report to the Centers for Medicare and Medicaid Services (CMS) all “transfers of value” to all Healthcare Professionals
(HCPs) attending this event, including transportation, lodging, and meals. Please be aware that CMS will post on a public website the name of each recipient, the dollar value of each item, the
date of each “transfer,” and other required information. For more information about reporting requirements, and for information on the Sunshine Act in general, please visit our website at http://

Please note that in accordance with Vermont Law, Vermont-licensed physicians must reimburse DJO Surgical for the full cost of any meals and refreshments immediately after the event. Any
physician with a Vermont license, even if that individual is not actively practicing in Vermont, must notify DJO of such licensure in advance of the event.
